Your Medical Records
The Health Information Management Department holds the medical records of all New England Baptist Hospital patients. We do not have records for private physicians or the Occupational Health Center.
Learn how to receive radiology images.
To get a copy of your medical records, you can send a letter of request to our office. Please include the following information in your letter:
- Name (please print)
- Date of birth
- Information requested (dates of service, reports)
- Your address
- Your signature
You can also access your medical records electronically through our convenient online patient portal.
Authorization to Release Protected Health Information
If you want your record sent to someone, you first have to fill out our Authorization to Release Protected Health Information form.
This form meets all relevant HIPAA requirements, including 45 CFR Parts 160 and 164 of the 1996 Health Insurance Portability and Accountability Act.
Please make you sure to answer all form prompts and allow enough time for our team to process your request. If you need your records for an upcoming appointment, please specify your timeframe so we can help you.
Unless your record is being sent directly to a physician, we charge a fee for this service. We will waive this fee if you’re only sending the record abstract.
